Job Description:
A company is looking for a Consultative Pharmacist 2 to provide
optimal medication therapy and patient care.
Key Responsibilities
Consults with patients, pharmacies, and providers to recommend
appropriate medication therapy
Documents interventions and maintains accurate patient profiles to
enhance care outcomes
Collaborates with healthcare professionals to create cost-effective
medication care plans
Qualifications, Training, and Education
PharmD or B.S. in Pharmacy from an accredited school of pharmacy
required
Experience with hospice care or oncology preferred
Must meet minimum requirements for the company's work-at-home
internet policy
Proficient in Microsoft Office applications
Strong commitment to patient care
