Job Description:
A company is looking for a Clinical Care Manager, Behavioral
Health.
Key Responsibilities
Connect with patients and care teams primarily via phone to improve
patient care
Conduct comprehensive assessments to identify patient needs and
create care plans
Coordinate care and serve as an advocate and resource for patients
and their families
Required Qualifications and Education
Master's Degree in Social Work (MSW) or Counseling
New York State Licensed Clinical Social Worker (LCSW) or Licensed
Mental Health Counselor (LMHC)
3-5 years of behavioral health experience, preferably in case
management
At least two years of community-based Behavioral Health
experience
Must be located in Washington or Alaska
